Can I pre-register for my initial consult?

ACSN is pleased to offer the ability for new patients to pre-register online.  This allows you to avoid some of the paperwork when you arrive at our office for your visit.  To pre-register, you can visit our Patient Portal and click on the "pre-register" link below the login area.

How do I access the patient portal?

ACSN is excited to offer patients access to your personal health information online, wherever you are!  You can access the Patient Portal here.

With patient portal you can request a new appointment, view your personal health record, receive educational materials, view current and past statements, receive health maintenance reminders, and pay bills (coming soon). 

To get started with patient portal, please contact our office and speak with your doctor's assistant.

What is my username and password for the patient portal?

If you are a first time user, you will need to register with the office to obtain your username and password.  Please contact the office and speak with your doctor's assistant to setup your account.

If you are an existing user and you have lost or forgotten your username and/or password, please contact our office to reset or unlock your account. 

What is Minimally Invasive Neurosurgery?

Minimally invasive surgery is any surgical procedure that is less invasive than open surgery. Minimally invasive surgeries typically require only a small incision, sometimes no more than an inch.  A minimally invasive procedure typically involves the use of a microscope or endoscope that is passed through a small opening in the skin. The endoscope contains a tiny camera that directs the surgeon and provides them with a “view” to the treatment area.

Doctors evaluate each patient's condition to determine the best approach to achieve the desired outcome. Provided the patient is in good general health, most back, neck, and other spine surgeries, such as spinal decompression and spinal fusion, can be performed using minimally invasive technologies including the METRx tube. Brain surgery can also be performed using minimally invasive technologies but it may be limited to certain types of conditions and pathology. Read more about minimally invasive surgery here.

How do I get to your main office?

Our main office is located at 716 S. Milwaukee Avenue in Libertyville, Illinois, approximately 30 miles north of Chicago. Click here for a map. When turning in from Milwaukee Avenue, please note that the road is divided! Please turn in on the south side of the median to avoid oncoming traffic.

How do I get my disability or FMLA forms filled out?

Always plan ahead. Please fax, email, or drop off the forms as early as possible. We do charge $25 for forms completion. Most forms are completed within one week. We may need to contact you for additional information, so please watch for calls from our office to keep things moving forward. Once completed by a provider, the form will be faxed to the recipient, or if requested, returned to you or made available for pickup.

Why do I need to schedule an appointment to follow up after an MRI?

At ACSN, we want to make sure you get the care you deserve in a timely manner. By scheduling an appointment when we receive your imaging, we are able to cut your wait time significantly. If the Neurosurgeon’s recommendation after reviewing your MRI is that you do not need to be evaluated by a Neurosurgeon then your appointment will be cancelled and you will not be charged.

If I need surgery, how quickly can I get on the surgical schedule?

The answer is, it depends. If your case is deemed an emergency, it will be treated immediately. If your procedure is elective, the timing will depend in large part on the pre-authorization requirements of your insurance carrier as well as your surgeon’s schedule. Some of our surgeons book out for 2-3 months while others have sooner availability. Our surgical schedulers will work with you to schedule your procedure.

What do I need to bring to my initial consultation?

You will need to bring a photo I.D., your insurance card (if applicable), workers compensation claim information (if applicable), and your most recent MRI/CT images and reports (if applicable). We also ask that you complete the patient registration forms prior to your appointment and submit them electronically. If you are unable to complete the forms electronically prior to your visit, please plan to arrive 15 minutes early for your appointment and bring all necessary health history to complete the forms in office.

Do I need to provide my health insurance information if I have a work-related injury or other type of accident that is in litigation?

Yes. We gather this information in the event your claim does not get approved or your litigation does not proceed as planned so that we may submit the claims to your insurance carrier. If you choose not to provide your health insurance information then you become personally responsible for 100% of the charges should worker’s compensation get denied.

What if I can’t pay my balance in full?

At ACSN, we understand that medical needs sometimes don’t align with financial means. Our patient accounts representatives work with qualifying patients to setup a payment plan. Please contact our patient accounts representatives at (847) 549-1910 when you receive your statement to discuss payment options. Please never ignore a bill as it will eventually get sent to our collection agency. We are happy to work with patients to establish a payment plan.

What languages are spoken in your office?

At ACSN, we have both staff and surgeons that speak English, Spanish, Russian, and French.

If I have questions for the doctor after my appointment, how do I get in touch with him/her?

Questions are welcome at any time. During normal business hours, you can contact our at (847) 362-1848 and choose the menu option for existing patients.

I need a refill on my prescription. Who do I contact?

Please contact your pharmacy and have them fax a refill request to our office at (847) 362-3351. Once received by our office, your prescription will be reviewed by a provider, and if approved, sent back to the pharmacy. If the provider feels that an appointment is needed or the refill is not approved, our staff will contact you. Prescription refill requests received after 4:00PM Monday - Friday will be reviewed the following business day. Requests received on Saturdays, Sundays, and holidays will be reviewed on the next business day. Prescription refills will not be handled outside of normal business hours.
